What Is Complex Care?

Complex care is specialist support provided to help people manage complex health and medical conditions which might include chronic or long-term health conditions terminal illnesses, life-limiting illnesses or specialist medical conditions that require extra and intensive Care and Support to manage their symptoms and day-to-day activities.

Exceptional care at home provides Complex care to help you or your loved one receive all the specialist care and support that you need to achieve independent living at home.

Complex care usually involves medical intervention with the involvement of clinicians, nurses and any other medical personnel who specialise in a particular Service User’s condition. Essentially complex care can include any condition that requires clinical support and it can also be provided if someone requires nursing care as result of their primary condition. The aim of complex care at home therefore is to provide support around an individual’s needs and enable them to retain their independence as much as possible.

We do understand that everyone’s care situation is entirely different. As such, our complex care packages are flexible, ranging from short to long periods of time, and they can be adapted to suit your changing needs. Whether you’ve just come out of hospital and need care for weeks or months to help you get you back on your feet, or if you have a complex condition that you’ve lived with since birth and want to be able to live by yourself, we are able to provide complex care for you.

Exceptional care at home’s complex care is personalised to the particular needs of a specific service user. Overall, our aim to enable our Service Users to continue doing the things they love to do in an environment they have grown accustomed to, closer to friends and family, and maintaining ties with the community.

Live-in care: Here we provide round-the-clock support and care to you, where a Carer lives with you in your home so that you can receive the care you need whenever you require it.

Visiting care: Here we provide care and support on an hourly basis when and how you need it. Visiting care can range from a few hours to many hours a day or weekly depending on your care needs.

Respite care: Here we provide temporary relief for a primary care giver. You may choose respite care if your regular caregiver needs to take a break from their care giving role to help them recharge.

Your complex care is based on a carefully designed support plan. This includes all details relating to your health care needs, including any diagnosis and treatment required. Your support plan is designed around your needs and wishes and it will include all important information such as their next of kin and other medical professionals involved in their care.

Your support plan also includes detailed information and instructions to carers. This ensures that the care they deliver is exactly as agreed with you or your loved one during the initial needs assessment. Your care plan can be altered or adapted to your changing care needs.

Your Complex support plan will also involve the input of clinical nurse who works closely with the care manager in carrying out the initial needs assessment to ensure all aspects of your clinical care requirements are catered for.
